Our
Responsibilities
You
will be treated as an individual and afforded courtesy and respect at all
times. You have the right to be treated confidentially.
Respect for religious
and cultural beliefs will be honoured.
We will answer our telephone promptly
and courteously.
You have a right to information about your health (illness
and treatment, possible side effects, prevention or recurring illness). You have
the right to see your own medical records subject to the limitations of the law.
A charge may be made.
Test results may be given over the telephone if you
call for them or you may be asked to make an appointment with the doctor to discuss
them. You may be asked for proof of identity.
When making an appointment
you will be offered a time to see a doctor in accordance with the system used
in the practice. If there is likely to be a substantial delay for any reason,
you will be given an explanation.
Routine referral letters for hospital
appointments will normally be despatched within three working days of the referral
being agreed with the doctor. Urgent referrals may be faxed, telephoned or provided
as a handwritten letter for the patient to take to hospital.
Home visits
will be made if requested and the doctor agrees you or the patient are unable
to attend the surgery. The final decision rests with the doctors.

We
ask that you treat our doctors and staff with courtesy and respect.
If you
are unable to keep an appointment please let us know so that we can offer this
appointment to someone else.
Please do not ask for a home visit if you or the patient are able to attend the surgery. If a home visit is necessary please call early in the morning.
Please acquaint yourself with the method of obtaining a repeat prescription.
Kindly leave routine telephone calls until later in the day if possible
as the first two hours of the morning can be extremely busy. If you cannot avoid
this time please keep the call brief.

Freedom Of Information - Publication Scheme
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available.
This scheme is available from the practice reception.
Complaints Procedure
We have a complaints procedure within the practice. If you have a complaint please raise it with a member of staff.
However, you are entitled to approach the Health Service Ombudsman at www.ombudsman.org.uk or ring their Complaints Helpline on 0345 015 4033. You are also entitled to approach
the Complaints Department at NHS Leeds.
Confidentiality
All information passed to the practice regarding yourself is treated in the strictest confidence and we are registered under the Data Protection Act. However, for your
effective care it is sometimes necessary that information is shared between members of the practice team. No information is passed to third parties without your written consent or, in the case of you being referred to
another healthcare provider, your consent at the time of the consultation.
Staff Protection
A zero tolerance policy towards violent, threatening and abusive behaviour is now in place throughout the National Health Service. The staff in this
practice have the right to do their work in an environment free from violent, threatening and abusive behaviour and everything will be done to protect that right.
If you do not respect the rights of our staff we may choose to inform the police
and make arrangements for you to be removed from our medical list.
