HOW DO I...
MAKE AN APPOINTMENT?


Appointments can be made by telephone or in person during reception office hours. A limited number of doctors' appointments are pre-bookable up to four weeks in advance. Nurses: up to six weeks in advance. Please make a separate appointment for each person. If you are unable to attend, please let us know. Children should be accompanied by a responsible adult.

OBTAIN A HOME VISIT?


Visits are only for patients who are not physically able to attend the surgery. Please ring before 10.00am when possible.

HOW DO I...
OBTAIN A REPEAT PRESCRIPTION?

These can only be obtained by written request, in person, by post or fax. PLEASE ALLOW TWO WORKING DAYS FOR COMPLETION.

DO NOT request prescriptions by telephone. It can lead to errors and block the telephone for urgent calls.

Please note that you may be asked to attend a medications review consultation with a doctor or nurse during your course of treatment.


HOW DO I...
REGISTER?

If you wish to join the practice you may register by completing a registration form. You will then be offered an invitation to attend an appointment for a health screening examination.

Patients are registered with the practice and have the right to express a preference to receive services from a particular health care professional, either generally or in relation to any particular condition. Do recognise, however, that there may be times and circumstances when that person may not be available.
