 Quality of care isn't about only finding a person you need most, but also seeing this person in a safe and effective environment.

Our consultations include:

  • Chronic Disease Management: 

Chronic diseases are diseases of long duration and generally slow progression. They include heart disease, stroke, chronic respiratory diseases and diabetes. The care of chronic disease in the community settings is mainly managed by the patients themselves who become experts in their own condition and its treatment.  We are inviting patients in for an annual appointment for a Collaborative Care and Support Planning review (CCSP) that provides collaborative  care approaches using a multidisciplinary approach between the patient, Nurses, GPs and Pharmacists​.

 

  • Mental Health Reviews:  

Our mental health reviews take place in a familiar setting. Personal needs may change over time, so it's important that any treatment is reviewed on a regular basis.  Engaging in the Five Year Forward View for Mental Health services within the NHS (NHS England 2017) incorporating best practice for our patients striving towards a future where people have access to high-quality care close to home, and they are able to exercise choice – reflecting true shared decision making and personalised care.

  • Learning Disability Reviews:  

At Park Edge Practice we understand that People with a learning disability are a very diverse group. We aim to ensure Accessibility Standards are maintained with optional guidance and information for registered carers to ensure the patient can perform where possible a full range of everyday tasks. During the annual review we aim to address any illnesses or chronic disease management required in addition to the learning disability review itself.

  • NHS Health Checks: 

Patients in the UK aged between 40-74 will be invited in for a review of their health usually every 5 years.  The NHS health check is designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.

  • Cervical Screening programme:

Cervical screening (a smear test) is available to women and people with a cervix aged 25 to 64 in England. All eligible people who are registered with a GP (as female) automatically receive an invitation by mail. Transgender men (assigned female at birth) do not receive invitations if registered as male with their GP, but are still entitled to screening if they have a cervix. The first invitation is sent to eligible people at the age of 24.5 years. People aged 25 to 49 receive invitations every 3 years. People aged 50 to 64 receive invitations every 5 years.

Monday to Friday from 8:20 - 11:00 am and 3:30 - 5:30 pm. If you think you need to be seen the same day please call before 10.30 if possible.
